REGISTER | 5th Street Dance

Welcome to 5th Street Dance! We are so excited to be starting this journey with you all.

Please fill out the registration form below to ensure that your child will have a spot in class this fall. Class days and times will be sent out before our season begins. If you have more than one child enrolling, please fill out a separate form for each child. Please list any other activities that might conflict with classes and include dates and times. Once set class times are sent out and you find there are conflicts, notify me immediately so I can try to accomodate you to the best of my ability.

As we do not have an online payment structure, registration payment and applicable fees must be dropped off or mailed at the time of registration. Checks can be made out to 5th Street Dance, with your child's name in the subject line.

There are two options to register with. For instant registration online, fill out the attached internet form and mail in your check. If you'd rather download the PDF form and mail it with your check, click on the attached link and print. A non-refundable registration fee of $25 is required at the time of sign up (this includes recital t-shirt). Students registered after the registration deadline (June 1, 2024) will be charged a $35 registration fee. Class placement will not occur until registration fee is paid.

Payment and all applicable fees must be sent in promptly with your registration. Your child's spot will not be secured until payment has been received.

Students 3 years- 5 years old will be in the Itty Bitty classes performing Ballet and Tumbling dances at recital.

Students 5 years- 6 years old will be in the Mini Movers classes performing Jazz and Lyrical dances at recital.

* 3-5 yr olds will take tumbling in the Itty Bitty Class

**Production Class tuition helps to covers the cost of the set and props**

Please note whether you are paying in full by September 1st, or paying monthly in your check memo.

Multiple Children Discount: Take $5 off for every child after the first full price child.

(Example: Child 1 = $45 Child 2 = $40 Child 3 = $35 Total = $120)
